Transportation

Some of our plan provide transportation services for up to 12 one-way trips to plan-approved locations every year.

Plan approved locations include:

• Doctor’s offices.
• Outpatient facilities/centers.
• Clinics.
• Other health care related locations.

Transportation services must be approved and arranged by utilizing the plan contracted transportation vendor. Modes of transportation may include car services such as Uber and Lyft, wheelchair van, or non-emergency medical transportation. A 40 mile maximum per one-way trip is allowed.

Transportation arrangements must be made at least two (2) business days in advance of your appointment to arrange and approve each transportation request with our vendor. Please refer to your Evidence of Coverage for additional information.
