Important Information About Your Enrollment
Your Capital Blue Cross HMO or PPO enrollment application will be subject to the approval of the Centers for Medicare & Medicaid Services (CMS), the federal agency that administers Medicare. Capital Blue Cross will notify you by mail to let you know if CMS has accepted or denied your enrollment application for Capital Blue Cross HMO or PPO coverage. If you have health benefits through your own or your spouse's employer, please contact your group administrator.
The plan offerings in which you are enrolling may not be available in the following contract year, because by law, plan sponsors can choose not to renew their contract with CMS or reduce their service area and CMS may also refuse to renew the contract resulting in a termination or non-renewal.
