Introducing Strive Health

We understand that managing care for conditions like end-stage renal disease (ESRD) and chronic kidney disease (CKD) can be stressful and complicated. That’s why Capital Blue Cross is working with Strive Health (Strive) to offer you a program that helps coordinate your care for these conditions.

When you schedule your first appointment with Strive Health, you’ll be supported by your own team of Kidney Heroes™, a compassionate group of nurse practitioners, care managers, and other professionals who specialize in kidney care. Your Kidney Heroes team will also work closely with your primary care provider (PCP) and nephrologist (kidney doctor) to keep them informed of your condition and the services provided to you.

How Strive Health can help

Your Kidney Heroes team will provide support so you can make the most informed decisions. They will help you:

  • Learn about kidney disease, what you can expect as a member of the program, and how you can best manage your overall health.
  • Get face-to-face1 and online education from dieticians, clinicians, and social workers to support your personal needs.
  • Benefit from more quality time with your care team and get your questions answered.
  • Connect with the right specialists to address any healthcare needs.

How to enroll

Using the Strive Health program is voluntary and at no cost to eligible members. If you are interested in joining or would like more information, contact Strive Health 24/7 at 717.345.9117 (TTY: 711).
