By participating in the 2026 Your Rewards program you have opportunities to lower disease risk factors and address chronic conditions such as high blood pressure or diabetes.

The program is comprised of health and wellness activities designed to address each member’s unique lifestyle and healthcare needs. Members can receive reward dollars to use at over 70,000 nationwide retailers when they attest to completing eligible activities. Reward dollar amounts vary and depend on the eligible activity.

All 2026 Your Rewards program activities must occur between January 1, 2026, and November 30, 2026. The program gets underway on January 1, 2026. Members have until November 30, 2026 to attest to healthcare activities completed during 2026. Rewards must be used by February 28, 2027.

Enrollment in this program is offered to all plan members. However, eligibility qualifications do apply. Eligibility is based in part on results from scientific clinical research and addresses specific medical situations or clinical conditions. Reward eligibility is substantiated by claims documentation.
