Capital BlueCross has made some coverage changes to help with your care during the Coronavirus epidemic. We’ve also included additional information to keep you and your family safe during this crisis.


Member Authorization Form

Use this form to allow a family member or friend to speak with Capital BlueCross on your behalf. You can print it out and mail it to the address listed on the instructions, or call the Member Services number on the back of your ID card and request the form be mailed to you.


Identity Theft: protect yourself

This information from Medicare.gov has tips to keep your personal information safe. Remember to never share your personal information over the phone, even if you are threatened with legal action. No health organization or law enforcement agency would make these threats over the phone.
